Social stigma has become a barrier for public health in high-risk populations. This is the case of heroin using women and mothers in Puerto Rico, who have had to live in exclusion, marginalization, and discrimination because of the social stigma associated with the consumption of drugs and its image. I used a qualitative methodology and discourse analysis to explore the experiences of five (5) mothers that used heroin, five (5) service providers and four (4) social support sources who were interviewed in depth. I discuss the double stigma of gender inequality and substance users that women suffer. Social stigma promotes the lack of adequate treatments for heroin using women in Puerto Rico.
