Practice in hospital institutions is pervaded by stand-offs mainly related to patient attention and care procedures. Such stand-offs and/or challenges pose us the issue of formalizing -via theoretical-practical articulation -of the psychoanalyst role in the institution, including intrinsically limits to treatments and the impossibilities and possibilities of the institutional life. Elaborating on concerns about the issue of payment for services provided at the Hospital, which have been discussed in the Clinic weekly meetings, it is intended to devise possible inclusion ways and effects as well as payment handling. The following questions set the north for this paper: What is going hand in hand for the patient and for the psychoanalyst when the latter either charges for the services or does not? Is it possible, from the point of view of psychoanalysis, to support free treatment? Also, is it possible to support indirectly paid treatment (via medical insurance settlement, for instance)?
