This study aimed at discussing the challenges faced by the psychologist in the Núcleo de Apoio à Saúde da Família (NASF) regarding the demands arising from consumption of psychoactive substances and the possibilities for action. With respect to challenges, it was pointed out the lack of professional updating and management; the professional belief that nothing can be done when it comes to alcohol and drugs; the referral as the only care action performed; and the education of psychologists still based on the liberal, private, curative and individual clinic model. We must create practices in accordance with the individual's context, as well as rescuing the multiple dimensions of health and incorporating other forms of knowledge so as to compose the production of health care, such as Harm Reduction, that recognizes each user in their uniqueness and devises strategies to promote health.
