From a brief history of therapeutic accompaniment, we try to draw some perspectives for this clinic in the Lacanian orientation. The concept of secretary is related to the ideas of intervention in the Other (Zenoni), stapling by the social and trivialization of delirium. Some clinical vignettes are presented in order to illustrate these formulations
