The purpose of this article was to investigate the play in the psychodynamic psychotherapy of a child with externalizing symptoms. A mixed design and a systematic case study approach were used. All sessions were filmed and evaluated with the Child Psychotherapy Q-Set (CPQ). The items related to playing were analyzed for their means and correlations with the time of treatment. The psychotherapist's report made up the qualitative analysis. During the initial and intermediate periods, the instruments indicated a resistant child, with intolerance to frustration, strong feelings of irritation and aggressiveness, as well as having difficulties in exploring content related to problems and behavioral oscillation - characteristics that were reflected in his play. The psychotherapist, on the other hand, showed to be affectively engaged, sensitive, and flexibilized the technique. Throughout the psychotherapy, the patient developed a greater engagement in the activities and a more fluid, spontaneous and symbolic play. The elaboration of internal contents and aggressive impulses was possible the, evidencing an improvement in the capacity for emotional regulation was observed. The changes in psychotherapy did not occur in a linear way, identifying evolution and regression movements throughout the process.
