The purpose of this article is to report an internship experience, developed in 2011, in a public health service in a medium-sized municipality, located in the interior of the State of São Paulo. This report to discuss the ways, actions and practices in workers' health at this institutional establishment. We state a historical view about the worker's health condition in Brazil, the advances, the (im) possibilities and the main conflicts faced by the workers who operate this public policy.
