The article was based on a PhD field experience at a CAPS III in Curitiba, to reflect on current mental health practices and their challenges, in a dialogue between history, epistemology and anthropology. The research approach was qualitative, descriptive, based on a field experience that used field diary and interviews. The centrality of care in the individual was noted often leaving aside their social and family networks, and a primary use of medicines as central therapy. It can be seen from this experience that the construction of the concept of disease is carried out from a naturalist epistemology, obeying a reductionist, explanatory and causal logic. The importance of a critical and reflexive view of the epistemological foundations that guide our practices is taken up.Phenomenology is presented as a possible epistemological alternative that can aid in a more complex understanding of psychopathological phenomena
