The psychological documentation in patient record enables the planning of appropriate intervention and is a source of research information. Although the record is one of the psychologists' skills, in general, this is poorly done. In order to describe how this record has been made in a psychology school clinic in the state of Sergipe was performed a documentary retrospective analysis of 119 psychological records from the years 2009 and 2010. As a result it was possible to characterize the clientele and to propose new forms of intervention. However, were found several problems in the records reviewed, which were classified according to their nature and grouped into seven problems categories: summary sheet, different information, signatures, lack of information, sheet closure, handwriting and lack of documentation. At the end, we concluded that the difficulty in record come since graduation course, being necessary to rethink ways of developing this competence.
