This article presents a critique to suspension of the ontological discussions in the DSM-V. The objective is to defend the importance of these theoretical discussions in the field of psychopathology using the Heidegger's ontological difference. First, it is intended to warn that the suspension of ontological reflections has the effect of reducing the psychopathological experience and the devaluation of the language and the clinic in favor of overvaluation of the material supports. Second, the absence of such discussions do not prevent an implicit ontology from operating and producing social effects. To illustrate this question, it is addressed the notion of the substance deficit to be restored, that is the basilar matrix of the organic etiological hypotheses. It is concluded that this matrix convokes for an ethics in the style of an essence to be fulfilled, that is coupled with the logic of consumption in a society that demands performance.
