The Health Family Strategy aims the promotion, prevention and the recovery of health and understands the family as a privileged forum for the implementation of an expanded concept of health. In this context, chronic diseases such as hypertension and diabetes mellitus, which constitute a global health problem with impact over personal and social context, should be properly addressed by the Health Family Strategy. This was an exploratory and qualitative study with the aim to identify perceptions of patients with hypertension and / or diabetes mellitus, assisted by a Health Family Nucleus in a city of Sao Paulo State. It was investigated the perception about the clinical picture, influence of emotional state over the health and its improvement. Sixty-seven patients were interviewed. Most verbalizations about symptoms of hypertension includes reports of pain, tachycardia, dizziness, impaired vision and digestive problems. About the emotional influence it was observed verbalizations associating the symptoms of hypertension with nervousness and anxiety. When asked to identify changes related to diabetes, most patients described symptoms such as weakness, sweating, difficulty with vision, pain in the head and legs and dizziness. They also associated nervousness to diabetes, followed by sadness. In general, patients reported they did not have options for leisure, although understood as a protective fator against emotional and physiological issues. Visiting family members was the most commonly leisure done.
