In this article a case study derived from a therapeutic process in order to analyze the relationship between parental role confusion in the process of individuation of a teenager adopted by the maternal grandmother, her biological mother within the family system remains . The analysis allows us to observe how the "symptoms" of the patient were within a family system dominated confusion, triangulations and fuzzy boundaries, among others. This family structure, together with the parental role confusion, where there is no clearly defined maternal figure, would hamper the process of individuation of the teenager
