The work consists of a systematic review of the literature, to assess the hypothesis that there would be a negative (inversely proportional) association between the perceived severity of HIV and exposure to HIV. Observational studies published between 1998 and 2018 were selected. 151 potentially relevant references were identified, 50 articles were chosen for full reading and, of these, 26 articles were selected for the synthesis of this research. In 17 studies, the evaluated hypothesis was not confirmed, against 9 studies in which an association was found between the variables. In the discussion of the findings, the operationalization of the analyzed constructs is problematized, the limits and the scope of the research designs adopted, as well as the ethical implications of health actions based on constructs, perceived severity, and risk behaviors. In conclusion, recommendations for HIV prevention strategies are proposed.
