The French psychiatrist Georges Lantéri-Laura used the concept of "paradigm", developed by T. S. Kuhn, to explain the rationality and history of psychopathology. In his essay on the paradigms of modern psychiatry, Lantéri-Laura established three paradigms, situated between the end of the 18th century and the end of the 20th century. The first paradigm has mental alienation as its center of the idea. The second works with the notion that mental illnesses exist. The third uses the structure notion and defines the major psychopathological structures. Thus, this theoretical essay had as its starting point the historical review of psychiatric traditions carried out by Lantéri-Laura, seeking to advance and include contemporary discussions in the proposal to defend a fourth psychiatric paradigm guided by the concept of the syndrome. In this study, it is possible to identify two quite distinct psychopathological logics - one psychoanalytic and the other behavioral - on which the Diagnostic and statistical manual of mental disorders (DSM), a manual of the American Psychiatric Association (APA) is based. For this, it was necessary to prepare a brief context of the DSM and its five editions on the main criticisms of this classification system. Finally, it discusses the fate of the DSM system in the face of a new emerging diagnostic system and concludes that psychopathology is a diachronic field in which changes and updates are constant and necessary.
