We sought to verify the relationships between predictors and effects associated with the openness variable and its relation with sexual and reproductive prevention of lesbians. A study was conducted with 1,146 Brazilian women self-identified as lesbians with a mean age of 23 years. The main hypothesis was that distress, general openness, openness to the health professional, and sociodemographic variables interfere in the sexual and reproductive prevention of this population. Data were collected using a self-administered online instrument, performing descriptive and inferential analysis, in addition to observing differences in belonging to groups. The results indicated that lesbians have little knowledge about preventive practices, and it is argued that the lack of openness/reliability interferes with the access of lesbians to health services, as well as the quality of care, which therefore affects their sexual and reproductive prevention. Thus, it is alerted to the need for preparation of health professionals to better understand the specific demands of lesbians, to then instruct them about prevention, besides providing a welcoming environment in the care.
