A total of 70 children and adolescents aged 7 to 14 years, with ASD (n=16), with RD (n=19) and with typical development (TD, n=35) participated. Controlling the effect of age, the results indicated superior performance of the ASD group. The DL group presented inferior performance than the other groups in Part A and in the total test score when compared to the DT group. The performance of the TEA group was discussed from the Theory of Weak Central Coherence, while the performance of the DL group, based on the hypothesis of multiple deficits in dyslexia and considering the impact of less access to written materials in visuospatial skills. Studies of this nature may help in the cognitive characterization of children / adolescents with ASD and DL.]]></p></abstract>

Intelig&ecirc;ncia fluida &eacute; a    capacidade de adaptar e flexibilizar o pensamento quando n&atilde;o existem    recursos classificados na mem&oacute;ria para responder a tarefas complexas    (Cattell, 1971). Tal capacidade inclui opera&ccedil;&otilde;es mentais, como    reconhecimento e forma&ccedil;&atilde;o de conceitos, resolu&ccedil;&atilde;o    de problemas, extrapola&ccedil;&atilde;o e transforma&ccedil;&atilde;o da informa&ccedil;&atilde;o.    Para avaliar intelig&ecirc;ncia fluida s&atilde;o utilizados testes n&atilde;o    verbais, uma vez que s&atilde;o menos dependentes da cultura e da linguagem    (Mungkhetklang, Crewther, Bavin, Goharpey, &amp; Parsons, 2016; Otero, 2017).    O Teste de Matrizes Progressivas Coloridas de Raven (MPCR) tem sido um dos testes    mais utilizados mundialmente na avalia&ccedil;&atilde;o desse construto em pesquisas,    inclusive no Brasil (Cardoso, Lopes, Oliveira, &amp; Braga, 2017; Wongupparaj,    Kumari, &amp; Morris, 2015).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Yoshida et al.    (2017) destacam o papel da percep&ccedil;&atilde;o visual no MPCR, com a Parte    A caracterizada por identidade, similaridade e diferen&ccedil;a, a Parte Ab,    por caracter&iacute;sticas de orienta&ccedil;&atilde;o espacial, como simetria    e localiza&ccedil;&atilde;o, e a Parte B, com princ&iacute;pios tanto espaciais    quanto de l&oacute;gica. Al&eacute;m de testar a percep&ccedil;&atilde;o visual    e o racioc&iacute;nio l&oacute;gico, o MPCR tamb&eacute;m &eacute; considerado    uma medida de habilidades visuoespaciais. Do ponto de vista neuropsicol&oacute;gico,    o teste envolve mem&oacute;ria de trabalho, flexibilidade mental, mudan&ccedil;a    de categoria, resolu&ccedil;&atilde;o de problemas, abstra&ccedil;&atilde;o    e racioc&iacute;nio (Kafadar &amp; Orhan, 2016), capacidades conhecidas como    fun&ccedil;&otilde;es executivas (Baggetta &amp; Alexander, 2016).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A intelig&ecirc;ncia    fluida tem sido alvo de pesquisa no &acirc;mbito do Transtorno do Espectro Autista    (TEA). Essa condi&ccedil;&atilde;o cl&iacute;nica &eacute; caracterizada por    d&eacute;ficits persistentes na intera&ccedil;&atilde;o e comunica&ccedil;&atilde;o    social e pela presen&ccedil;a de padr&otilde;es restritos e repetitivos de comportamentos,    interesses ou atividades (<i>American Psychiatric Association</i> [APA], 2013),    embora exista uma ampla variabilidade na manifesta&ccedil;&atilde;o de sintomas.    Por outro lado, as habilidades visuoespaciais e a intelig&ecirc;ncia fluida    tem, nas &uacute;ltimas d&eacute;cadas, constitu&iacute;do uma potencialidade    cognitiva no autismo (Dawson, Souli&egrave;res, Gernsbacher, &amp; Mottron,    2007). Por exemplo, estudos realizados no Canad&aacute; mostraram que crian&ccedil;as    e adolescentes com TEA, entre sete e 16 anos (<i>n </i>= 38, Dawson et al.,    2007), e na faixa de 11 anos (<i>n </i>= 25, Nader et al., 2014), apresentaram    escore superior no MPCR se comparado &agrave; medida de QI de Wechsler. Os referidos    estudos, por&eacute;m, n&atilde;o compararam os escores nas diferentes partes    (A, Ab e B) do MPCR, apenas no escore geral.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A Teoria da Fraca    Coer&ecirc;ncia Central (Happ&eacute; &amp; Frith, 2006) sup&otilde;e que o    autismo &eacute; caracterizado por um tipo de processamento que privilegia as    partes do est&iacute;mulo ao inv&eacute;s do todo, com dificuldade de processar    e integrar o contexto. Nessa vis&atilde;o, a potencialidade dos indiv&iacute;duos    com TEA em tarefas de racioc&iacute;nio abstrato visuoespacial ocorre devido    a um processamento visuoespacial que privilegia as partes do est&iacute;mulo,    com dificuldade de processar o est&iacute;mulo como um todo integrado. Em outras    palavras, indiv&iacute;duos com autismo tendem a perceber a cena visual como    um conjunto de detalhes fragmentados e n&atilde;o como uma unidade congruente    (Happ&eacute; &amp; Frith, 2006), apresentando vantagem em tarefas de busca    visual (ver Kaldy, Giserman, Carter, &amp; Blaser, 2016, para revis&atilde;o).    Assim, parece que alguns indiv&iacute;duos com TEA podem obter melhor desempenho    em tarefas que priorizem as partes (processamento local) sem forte depend&ecirc;ncia    do contexto para resolu&ccedil;&atilde;o do problema.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Corroborando essa    vis&atilde;o, um estudo de meta-an&aacute;lise (Muth, H&ouml;nekopp, &amp; Falter    2014) investigou se o processamento visuoespacial no TEA &eacute; superior,    inferior ou igual ao de n&atilde;o autistas, tendo como uma das teorias de base    a Teoria da Fraca Coer&ecirc;ncia Central. Foram inclu&iacute;das 76 amostras    e os dados foram tratados no software <i>Comprehensive Meta Analysis</i> 2.2.    Os resultados indicaram desempenho superior de indiv&iacute;duos com TEA em    testes como <i>block design</i> (<i>d</i> = 0,32) e <i>figure disembedding</i>    (<i>d </i>= 0,26), com mais forte prefer&ecirc;ncia pelo processamento local    na tarefa visuoespacial <i>navon task</i> (tarefa que consiste na apresenta&ccedil;&atilde;o,    por exemplo, de uma letra em tamanho grande composta por c&oacute;pias da outra    letra em tamanho pequeno e formato diferente). A prefer&ecirc;ncia pelo processamento    local tamb&eacute;m foi observada no desempenho de 21 crian&ccedil;as italianas    com TEA em uma tarefa modificada de <i>block design</i> (Cardillo, Menazza,    &amp; Mamarella, 2018).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Considerando-se    que as partes iniciais do MPCR avaliam rela&ccedil;&otilde;es simples de identidade,    similaridade e diferen&ccedil;a, com processamento semelhante ao local, uma    hip&oacute;tese &eacute; de que indiv&iacute;duos com TEA apresentem maior desempenho.    Corroborando essa hip&oacute;tese, o estudo de Courchesne, Meilleur, Poulin-Lord,    Dawson e Souli&egrave;res (2015), realizado com 30 crian&ccedil;as canadenses    com TEA, entre seis e 12 anos, com idade escolar m&iacute;nima e elevados n&iacute;veis    de comprometimento adaptativo, verificou que 56,7% das crian&ccedil;as alcan&ccedil;aram    escores no MPCR equivalentes a um QI de 75 ou maior.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A intelig&ecirc;ncia    fluida tamb&eacute;m tem sido estudada no diagn&oacute;stico de transtornos    de leitura (DL) em crian&ccedil;as e adolescentes. Ainda que essas dificuldades    sejam tipicamente descritas como um transtorno de linguagem, em que o dom&iacute;nio    fonol&oacute;gico est&aacute; prejudicado (Peterson &amp; Pennington, 2015),    o QI preservado tem sido um crit&eacute;rio crucial para a defini&ccedil;&atilde;o    do quadro (APA, 2013). H&aacute; autores que advogam a favor de uma compreens&atilde;o    mais ampla do perfil neurocognitivo de pessoas com dislexia. Gilger (2017),    por exemplo, discorre sobre as habilidades n&atilde;o verbais/espaciais. Em    revis&atilde;o recente da literatura, o desempenho dos disl&eacute;xicos parece    igual ou inferior ao do grupo de leitores com desenvolvimento t&iacute;pico    (DT) em uma variedade de tarefas visuoespaciais (Gilger, Allen, &amp; Castillo,    2016). H&aacute; uma evidente heterogeneidade no perfil neurocognitivo de crian&ccedil;as    com DL (Willems, Jansma, Blomert, &amp; Vaessen, 2016). Um dos quatro subtipos    de perfis encontrados por Willems et al. (2016) &eacute; o grupo chamado "<i>general    poor readers</i>", que apresentam desempenho baixo em todas as habilidades cognitivas    relacionadas &agrave; leitura e nas habilidades cognitivas gerais (QI).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Dessa forma, as    habilidades de racioc&iacute;nio n&atilde;o verbal (como as avaliadas pelo MPCR)    podem afetar o desenvolvimento das habilidades de compreens&atilde;o de leitura    (van Wingerden, Segers, van Balkom &amp; Verhoeven, 2018). Testando um modelo    de equa&ccedil;&otilde;es estruturais van Wingerden et al. (2018) verificaram    que as habilidades consideradas fundamentais para alfabetiza&ccedil;&atilde;o    (consci&ecirc;ncia fonol&oacute;gica e rela&ccedil;&atilde;o grafema-fonema)    juntamente com o racioc&iacute;nio n&atilde;o verbal (MPCR) exerceram um efeito    direto sobre a compreens&atilde;o de leitura em crian&ccedil;as com leve defici&ecirc;ncia    intelectual (m&eacute;dia de QI = 60,38; todas as crian&ccedil;as tinham um    QI estimado acima de 50 pontos e pelo menos alguma capacidade de leitura), al&eacute;m    de decodifica&ccedil;&atilde;o, compreens&atilde;o auditiva e compreens&atilde;o    pr&eacute;via de leitura. Os autores conclu&iacute;ram que habilidades de racioc&iacute;nio    n&atilde;o verbal s&atilde;o necess&aacute;rias &agrave; compreens&atilde;o    da leitura.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Mesmo em crian&ccedil;as    sem rebaixamento intelectual, o racioc&iacute;nio n&atilde;o verbal explica    em torno de 5% da vari&acirc;ncia em compreens&atilde;o de leitura, adicionalmente    &agrave;s habilidades de decodifica&ccedil;&atilde;o, compreens&atilde;o auditiva    e compreens&atilde;o de leitura pr&eacute;via (Oakhill &amp; Cain, 2012). Em    estudo longitudinal, Oakhill e Cain (2012) mostraram que o QI n&atilde;o verbal    contribuiu para o desempenho em compreens&atilde;o de leitura via habilidades    de monitoramento (metacognitivas), QI verbal, vocabul&aacute;rio e compreens&atilde;o    da estrutura da hist&oacute;ria.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Pode ent&atilde;o    haver diferen&ccedil;as de desempenho em intelig&ecirc;ncia fluida (n&atilde;o    verbal) entre crian&ccedil;as com e sem DL. No estudo de Salles e Parente (2006),    crian&ccedil;as de escolas p&uacute;blicas brasileiras, do 2&deg; ano (atual    3&ordm; ano do ensino fundamental), foram avaliadas no MPCR. O desempenho do    grupo de 2&ordf; s&eacute;rie competente em leitura e escrita (<i>n </i>= 15)    foi superior ao do grupo de 2&ordf; s&eacute;rie com DL e escrita (<i>n </i>=    14), apesar de todas as crian&ccedil;as apresentarem ao menos desempenho intelectualmente    m&eacute;dio no referido teste. Uma hip&oacute;tese &eacute; de que a DL tende    a manter as crian&ccedil;as afastadas dos materiais escritos, com menor h&aacute;bito    de leitura e escrita, tornando-as menos propensas a desenvolver habilidades    que tamb&eacute;m s&atilde;o exigidas para desempenhar o MPCR, como racioc&iacute;nio    l&oacute;gico, infer&ecirc;ncias, habilidades de resolver problemas complexos    e habilidades perceptivas visuais (Cruz-Rodrigues et al., 2014).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Mesmo que nas DL    existam d&eacute;ficits no dom&iacute;nio de processamento fonol&oacute;gico,    sabe-se que a leitura &eacute; um processo complexo que envolve tamb&eacute;m    processos sensoriais auditivos, habilidades de mem&oacute;ria, atencionais e    habilidades visuoespaciais (Carroll, Solity, &amp; Shapiro, 2015). Uma recente    meta-an&aacute;lise (Chamberlain et al., 2018), conduzida no software R, pacote    <i>metafor</i> (Viechtbauer, 2010), mostrou que a maioria dos indiv&iacute;duos    com dislexia (<i>N</i> = 956) apresentou um desempenho mais baixo em tarefas    visuoespaciais (ex.: testes de rota&ccedil;&atilde;o mental, <i>block design,    embedded figures</i>) se comparados a n&atilde;o disl&eacute;xicos (<i>N</i>    = 909). H&aacute;, por&eacute;m, estudos isolados que n&atilde;o encontraram    esses d&eacute;ficits na dislexia (ex., Ramus et al., 2003).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">No caso do teste    MPCR, os resultados s&atilde;o inconsistentes, com evid&ecirc;ncias sugerindo    desempenho inferior desse grupo cl&iacute;nico em crian&ccedil;as brasileiras    (Salles &amp; Parente, 2006) e chinesas (Tong &amp; Fu, 2013), apesar de evid&ecirc;ncias    contr&aacute;rias (ex., Tr&auml;ff, Desoete, &amp; Passolunghi, 2017). Na meta-an&aacute;lise    conduzida por Tong e Fu (2013), utilizando o<i> software</i> <i>RevMen</i> 4.2.2.,    crian&ccedil;as chinesas com DL, ainda que acima da m&eacute;dia em QI, apresentaram    escore total mais baixo no MPCR se comparadas aos pares com DT.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Em s&iacute;ntese,    h&aacute; evid&ecirc;ncias de que crian&ccedil;as com TEA podem apresentar bom    desempenho no MPCR (ex., Courchesne et a., 2015). Por outro lado, dificuldades    nas habilidades visuoespaciais podem ser encontradas em crian&ccedil;as com    DL, o que sugere uma poss&iacute;vel dissocia&ccedil;&atilde;o entre o desempenho    TEA e DL. Investigar essa dissocia&ccedil;&atilde;o &eacute; importante porque    auxilia na compreens&atilde;o das potenciais rela&ccedil;&otilde;es entre habilidades    visuoespaciais e processamento visual local. Estudos pr&eacute;vios (ex., Nader    et al., 2014; Tong &amp; Fu, 2013) analisaram o desempenho desses grupos cl&iacute;nicos    considerando somente o escore total/ percentil no MPCR. Analisar o desempenho    nas diferentes etapas do teste poder&aacute; fornecer informa&ccedil;&otilde;es    detalhadas sobre o processamento visuoespacial no TEA e na DL. Assim, o objetivo    do estudo foi comparar o desempenho de crian&ccedil;as com TEA e crian&ccedil;as    com DL, e de crian&ccedil;as com DT nas diferentes partes (A, Ab e B) do MPCR    e no total. A principal hip&oacute;tese foi de que, com base na literatura te&oacute;rica    e emp&iacute;rica, crian&ccedil;as com TEA apresentariam desempenho superior    aos demais grupos devido a um estilo cognitivo que privilegia o processamento    local.</font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>M&eacute;todo</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Participantes</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O delineamento    do estudo adotou a compara&ccedil;&atilde;o de grupos independentes. A amostra    foi composta por 70 crian&ccedil;as e adolescentes com idades entre sete e 14    anos, divididos em tr&ecirc;s grupos. Um grupo de 16 participantes com TEA (idade:    <i>M</i> = 9,87; <i>DP</i> = 1,14; 15 meninos/Grupo TEA), um grupo de 19 participantes    com DL (Idade: <i>M</i> = 10,47; <i>DP</i> = 1,39; seis meninos /Grupo DL) e    35 participantes com DT (Idade: <i>M </i>= 9,37; <i>DP</i> = 1,40; 25 meninos/Grupo    DT). N&atilde;o houve diferen&ccedil;as entre os grupos quanto &agrave; idade    (<i>p</i> &gt; 0,05), mas sim quanto ao sexo (<i>p</i> &lt; 0,01), com maior    n&uacute;mero de meninos no grupo TEA e de meninas no grupo DL.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">As crian&ccedil;as    e adolescentes com DL foram oriundos de escolas p&uacute;blicas e indicados    por seus professores. Para reduzir o vi&eacute;s do crit&eacute;rio de indica&ccedil;&atilde;o    do professor, outro crit&eacute;rio de inclus&atilde;o foi estabelecido que    foi apresentar escore menor ou igual a um desvio-padr&atilde;o da m&eacute;dia    do grupo normativo no Instrumento de Avalia&ccedil;&atilde;o de Leitura de Palavras    e Pseudopalavras Isoladas (LPI, Salles, Piccolo, Zamo, &amp; Toazza, 2013; Salles,    Piccolo, &amp; Min&aacute;, 2017). As crian&ccedil;as e os adolescentes com    TEA tiveram o diagn&oacute;stico confirmado de acordo com o crit&eacute;rio    do Manual Diagn&oacute;stico e Estat&iacute;stico dos Transtornos Mentais -    DSM-IV-TR (APA, 2000) e n&atilde;o possu&iacute;am rebaixamento intelectual.    Uma parte dos participantes foi selecionada por conveni&ecirc;ncia de uma amostra    do banco de dados de um programa de atendimento a indiv&iacute;duos do espectro    autista de um hospital geral. A outra parte foi constitu&iacute;da por crian&ccedil;as    e adolescentes que receberam o diagn&oacute;stico de TEA em programa de atendimento    &agrave; comunidade. As crian&ccedil;as com DT foram recrutadas de escolas p&uacute;blicas    e n&atilde;o foram identificadas como tendo DL (escore superior ao ponto de    corte em leitura, no LPI) ou TEA. Em todos os grupos de crian&ccedil;as (incluindo    o grupo de crian&ccedil;as com DL indicadas pelo professor), foram crit&eacute;rios    de inclus&atilde;o (conforme ficha de dados sociodemogr&aacute;ficos e de informa&ccedil;&otilde;es    cl&iacute;nicas): aus&ecirc;ncia de hist&oacute;rico de doen&ccedil;as neurol&oacute;gicas    ou psiqui&aacute;tricas (epilepsia, traumas, meningite, epis&oacute;dio convulsivo    e dist&uacute;rbios do sono), n&atilde;o usar medica&ccedil;&otilde;es e aus&ecirc;ncia    de dificuldades auditivas e visuais n&atilde;o corrigidas.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Instrumentos</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Ficha de dados    demogr&aacute;ficos e informa&ccedil;&otilde;es cl&iacute;nicas</b>. Pais ou    respons&aacute;veis de crian&ccedil;as e adolescentes de todos os grupos responderam    a um question&aacute;rio com 60 quest&otilde;es que avaliam aspectos como hist&oacute;ria    acad&ecirc;mica e condi&ccedil;&otilde;es gerais de sa&uacute;de. A vers&atilde;o    do question&aacute;rio para pais do grupo com TEA incluiu quest&otilde;es relacionadas    &agrave; tr&iacute;ade diagn&oacute;stica (preju&iacute;zos em comunica&ccedil;&atilde;o,    preju&iacute;zos em intera&ccedil;&atilde;o social e comportamento repetitivo).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Tarefa de Leitura    de Palavras/Pseudopalavras Isoladas - LPI</b> (Salles, Piccolo, Zamo, &amp;    Toazza, 2013; Salles et al., 2017). Avalia a precis&atilde;o na leitura oral    de palavras (e pseudopalavras) isoladas, que variam em suas caracter&iacute;sticas    psicolingu&iacute;sticas de regularidade (est&iacute;mulos regulares e irregulares),    lexicalidade (palavras reais e pseudopalavras), extens&atilde;o (est&iacute;mulos    curtos e longos) e frequ&ecirc;ncia de ocorr&ecirc;ncia na l&iacute;ngua (palavras    frequentes e n&atilde;o frequentes). O teste consiste de 60 est&iacute;mulos,    sendo 20 de cada categoria (palavras regulares, irregulares e pseudopalavras).    Para a pontua&ccedil;&atilde;o, s&atilde;o somados os acertos totais (m&aacute;ximo    = 60 pontos) e nas categorias palavras reais regulares (m&aacute;ximo = 20 pontos)    e irregulares (m&aacute;ximo = 20 pontos) e pseudopalavras (m&aacute;ximo =    20 pontos). A tarefa LPI foi normatizada em uma amostra de 419 crian&ccedil;as,    com idades entre seis e 12 anos, do 1&deg; ano ao 7&deg; ano do ensino fundamental,    de escolas p&uacute;blicas e privadas do Rio Grande do Sul (Salles et al., 2013).    Possui evid&ecirc;ncias de validade de construto (Sbicigo, Piccolo, Lima, &amp;    Salles, 2017), com correla&ccedil;&otilde;es entre fortes e moderadas com os    subtestes de leitura, escrita e artim&eacute;tica do Teste de Desempenho Escolar    (Stein, 1994), flu&ecirc;ncia de leitura no n&iacute;vel de palavras e pseudopalavras    (Justi &amp; Roazzi, 2012), tarefa de supress&atilde;o de fonemas (Lopes-Silva,    Moura, J&uacute;lio-Costa, Haase, &amp; Wood, 2014) e subteste vocabul&aacute;rio    da <i>Wechsler Abbreviated Scale of Intelligence</i> - WASI (The Psychological    Corporation, 1999; Trentini, Yates, &amp; Heck, 2014). Al&eacute;m disso, a    LPI discriminou o desempenho de crian&ccedil;as conforme a escolaridade, tipo    de escola, n&iacute;vel socioecon&ocirc;mico e dificuldades de aprendizagem    (Sbicigo, Lima, &amp; Salles, 2017). A confiabilidade observada na amostra normativa    foi alta (</font><font size="2">&#945;</font><font face="Verdana, Arial, Helvetica, sans-serif" size="2">    = 0,93).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Teste de Matrizes    Progressivas Coloridas de Raven - MPCR</b> (Angelini, Alves, Cust&oacute;dio,    Duarte, &amp; Duarte, 1999; Raven, Court &amp; Raven, 1986), Consiste em um    caderno de aplica&ccedil;&atilde;o com 36 figuras a serem completadas: 12 figuras    na parte A, 12, na parte Ab e 12, na parte B. Em cada uma das figuras, uma parte    fica oculta. A crian&ccedil;a deve escolher dentre seis alternativas qual delas    corresponde &agrave; parte que falta. Os itens s&atilde;o apresentados um a    um e organizados em tr&ecirc;s s&eacute;ries (Parte A, Parte Ab e Parte B) de    12 itens com dificuldades crescentes. Na primeira s&eacute;rie (Parte A), os    sujeitos precisam completar a parte ausente de um padr&atilde;o cont&iacute;nuo    com o padr&atilde;o id&ecirc;ntico ou sequencial, enquanto as partes Ab e B    envolvem analogias, permuta&ccedil;&atilde;o, altera&ccedil;&atilde;o de padr&atilde;o    e rela&ccedil;&otilde;es l&oacute;gicas. O MPCR foi padronizado em uma amostra    representativa de crian&ccedil;as (<i>N</i> = 1547) da cidade de S&atilde;o    Paulo, com idades variando entre 5 a 11 &frac12; anos, de escolas p&uacute;blicas    (municipais e estaduais) e privadas. A amostra foi dividida em 14 faixas de    idade, variando de 4 anos e 9 meses a 11 anos e 9 meses, cada faixa com amplitude    de seis meses. A confiabilidade do instrumento foi satisfat&oacute;ria por meio    do m&eacute;todo de duas metades. Crian&ccedil;as com pontua&ccedil;&otilde;es    indicando defici&ecirc;ncia intelectual foram exclu&iacute;das da an&aacute;lise    de dados.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Procedimentos</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Os estudos dos    quais derivaram os dados dos tr&ecirc;s grupos de crian&ccedil;as/adolescentes    no MPCR foram aprovados pelo Comit&ecirc; de &Eacute;tica do Instituto de Psicologia    da Universidade Federal do Rio Grande do Sul (UFRGS; n.&ordm; 2008067; n.&ordm;    2011/031). Ap&oacute;s a obten&ccedil;&atilde;o do Termo de Consentimento Livre    e Esclarecido assinado pelos pais e do consentimento das crian&ccedil;as, o    MPCR foi aplicado coletivamente nas escolas e tamb&eacute;m foi aplicado o LPI    para identifica&ccedil;&atilde;o de DL. As crian&ccedil;as/adolescentes com    TEA foram avaliadas com o MPCR em um &uacute;nico encontro. A avalia&ccedil;&atilde;o    foi individual e em sala privativa com padr&otilde;es adequados de ilumina&ccedil;&atilde;o    e condi&ccedil;&otilde;es de sil&ecirc;ncio, tanto no Instituto de Psicologia    da universidade como nas institui&ccedil;&otilde;es educacionais ou de atendimento    &agrave; comunidade. O tempo de aplica&ccedil;&atilde;o do teste foi de aproximadamente    45 minutos nos tr&ecirc;s grupos. As crian&ccedil;as/adolescentes do grupo com    DL e aquelas com DT, em geral, foram avaliadas no MPCR, em grupo de, no m&aacute;ximo,    nove crian&ccedil;as.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>An&aacute;lise    de Dados</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Foi utilizada estat&iacute;stica    n&atilde;o param&eacute;trica uma vez que os dados n&atilde;o apresentaram distribui&ccedil;&atilde;o    normal (<i>Kolmogorov - Smirnov</i> &lt; 0,05). As estat&iacute;sticas descritivas    consistiram no c&aacute;lculo da mediana e do primeiro e terceiro quartil no    MPCR para o grupo com TEA, DL e DT. Foi considerado o n&uacute;mero de acertos    das crian&ccedil;as/adolescentes nas tr&ecirc;s partes do teste e tamb&eacute;m    no percentil. As diferen&ccedil;as entre os grupos no MPCR (Parte A, Parte Ab,    Parte B, Total e Percentil) foram analisadas com o teste de <i>Kruskal-Wallis</i>,    com <i>post hoc</i> de <i>Dunn</i>, ajustado com a corre&ccedil;&atilde;o de    <i>Bonferroni</i>. Considerando a poss&iacute;vel influ&ecirc;ncia da idade    (desenvolvimento cognitivo) no desempenho no teste, foi realizada a An&aacute;lise    de Covari&acirc;ncia de Ranks de Quade (1967), utilizando <i>ranks</i> e res&iacute;duos    da an&aacute;lise de regress&atilde;o (IBM Corp, 2012), em que o efeito da idade    foi controlado.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Resultados</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A <a href="/img/revistas/avp/v18n2/11t1.jpg">Tabela    1</a> apresenta dados descritivos do desempenho dos grupos TEA, DL e DT no MPCR    e as an&aacute;lises de diferen&ccedil;as entre os grupos. Houve diferen&ccedil;as    significativas apenas na Parte A do teste. Compara&ccedil;&otilde;es com o teste    <i>post hoc</i> de <i>Dunn</i> indicaram que as diferen&ccedil;as foram entre    os grupos com TEA e com DL, e entre os grupos com TEA e com DT. Em ambos os    casos, o grupo com TEA apresentou desempenho significativamente superior. Outra    diferen&ccedil;a significativa foi entre o grupo com DT e com DL quanto &agrave;    classifica&ccedil;&atilde;o em percentil, em que o grupo com DL apresentou percentil    inferior.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A <a href="/img/revistas/avp/v18n2/11f01.jpg">Figura    1</a> mostra o desempenho dos grupos comparativo e cl&iacute;nicos no MPCR -    Parte A em diagrama <i>boxplot.</i> Como pode ser observado, o grupo de crian&ccedil;as    com TEA teve seus escores concentrados no quartil superior do diagrama.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O c&aacute;lculo    de ANCOVA de <i>ranks</i>, controlando a idade, indicou diferen&ccedil;as entre    os tr&ecirc;s grupos na Parte A do MPCR, ao inv&eacute;s de diferen&ccedil;as    apenas entre o grupo com TEA e os demais grupos, <i>F</i>(2,67) =18,71, <i>p    </i>&lt; 0,01, </font><font size="2"><i>&#951;</i></font><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i><sub>2    </sub></i>= 0,36. Diferen&ccedil;as tamb&eacute;m foram observadas na Parte    B, <i>F</i>(2,67) = 3,80, <i>p </i>&lt; 0,01, </font><font size="2"><i>&#951;</i></font><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i><sub>2    </sub></i>= 0,12, e no MPCR Total, <i>F</i>(2,67) = 3,53, <i>p </i>&lt; 0,01,    </font><font size="2"><i>&#951;</i></font><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>2    </i>= 0,09, enquanto as diferen&ccedil;as no percentil passaram a ser n&atilde;o    significativas,<i> F</i>(2,67) = 2,73, <i>p </i>= 0,07, </font><font size="2"><i>&#951;</i></font><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>2    </i>= 0,07. Compara&ccedil;&otilde;es com testes <i>post hoc</i> mantiveram    o desempenho superior do grupo com TEA sobre os demais grupos (<i>p </i>&lt;    0,01), acrescentando o desempenho superior do grupo com DT em rela&ccedil;&atilde;o    ao grupo com DL. Na Parte B, a signific&acirc;ncia foi marginal (<i>p </i>=    0,057) em rela&ccedil;&atilde;o a um desempenho superior do grupo com DT se    comparado ao grupo com TEA. No MPCR Total, o grupo com DT apresentou desempenho    superior ao do grupo com DL (<i>p </i>&lt; 0,05).</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Discuss&atilde;o</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O presente estudo    comparou uma amostra de crian&ccedil;as com diagn&oacute;stico de TEA e DL no    MPCR, comparando com crian&ccedil;as com DT. A hip&oacute;tese do estudo foi    que o grupo com TEA apresentaria um desempenho superior aos demais grupos. Essa    expectativa foi parcialmente corroborada, pois o grupo com TEA apresentou desempenho    superior ao grupo com DL e ao grupo com DT apenas na Parte A do teste. O desempenho    foi equivalente ao do grupo com DT na parte Ab, no escore total e no percentil    do MPCR. O desempenho superior do grupo TEA em rela&ccedil;&atilde;o aos demais    grupos na Parte A corrobora alguns estudos pr&eacute;vios em que houve superioridade    de participantes com TEA em uma gama de tarefas visuoespaciais que n&atilde;o    demandam sobrecarga cognitiva (ex., Barendse et al., 2013; Williams, Goldstein,    &amp; Minchew, 2006). Esse &eacute; o caso da Parte A do teste, na qual o sujeito    avaliado deve, predominantemente, perceber diferen&ccedil;as, similaridades    e simetrias em figuras cont&iacute;nuas sem depender de uma an&aacute;lise global    de contexto. Os resultados sugerem a necessidade de uso de testes de intelig&ecirc;ncia    n&atilde;o verbal em TEA, pois um potencial cognitivo criteriosamente avaliado    poder&aacute; ter implica&ccedil;&otilde;es importantes na educa&ccedil;&atilde;o    de crian&ccedil;as com o transtorno. De fato, o estudo de Courchesne et al.    (2015) revelou, em um grupo de 26 crian&ccedil;as com TEA e repert&oacute;rios    verbais reduzidos, avaliadas com o MPCR, que se elas tivessem sido avaliadas    exclusivamente com o WISC IV, as mesmas teriam corrido o risco de serem subestimadas    quanto ao seu potencial cognitivo.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A Parte A &eacute;    composta por itens que envolvem o complemento de padr&otilde;es simples e cont&iacute;nuos    (itens A1 a A8) ou complemento de um padr&atilde;o mostrando altera&ccedil;&otilde;es    progressivas em uma (itens A9 e A10) ou duas dire&ccedil;&otilde;es (A11 e A12),    todos com percep&ccedil;&otilde;es relacionadas &agrave; diferen&ccedil;a, identidade,    similaridade, orienta&ccedil;&atilde;o e forma gest&aacute;ltica (Angelini et    al., 1999). Considerando a hip&oacute;tese de que indiv&iacute;duos com TEA    possam apresentar fraca coer&ecirc;ncia central (Happ&eacute; &amp; Frith, 2006),    &eacute; poss&iacute;vel que o desempenho superior nessa parte do teste seja    explicado pela menor depend&ecirc;ncia do contexto na resolu&ccedil;&atilde;o    dessa etapa do teste, que envolve a resolu&ccedil;&atilde;o de padr&otilde;es    mais simples. Seguindo essa vis&atilde;o, a tend&ecirc;ncia de desempenho rebaixado    das crian&ccedil;as com TEA na Parte B seria explicada pela maior necessidade    de processar aspectos globais mais sofisticados e complexos. Os itens dessa    parte envolvem, de maneira discreta, o racioc&iacute;nio completo e coerente    por analogia espacial com uma mudan&ccedil;a sim&eacute;trica em uma figura    mudada e orienta&ccedil;&atilde;o obl&iacute;qua da parte faltante (itens B6    &agrave; B9). Os itens finais (B10, B11 e B12), por exemplo, envolvem um racioc&iacute;nio    abstrato pela analogia l&oacute;gica (Angelini et al., 1999).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Alternativamente,    essa tend&ecirc;ncia observada na Parte B poderia ser explicada por dificuldades    em diversas fun&ccedil;&otilde;es executivas em crian&ccedil;as com TEA (Czermainski,    Riesgo, Guimar&atilde;es, Salles, &amp; Bosa, 2014), pois essa etapa do MPCR    envolve a resolu&ccedil;&atilde;o de problemas complexos possivelmente exigindo    maior flexibilidade cognitiva e mem&oacute;ria de trabalho. A parte B do teste    cont&eacute;m tarefas que exigem um estilo cognitivo global e contextual a cada    conjunto de itens (Ben-Yosef, Anaki, &amp; Golan, 2017). Os dados encontrados    sugerem que, sempre que poss&iacute;vel, sejam verificados indicadores de habilidades    de flexibilidade cognitiva em idades precoces no TEA. Considerando alguns achados    recentes que mostram o valor preditivo e progn&oacute;stico dessa habilidade    para o funcionamento adaptativo em idades posteriores, o teste MPCR pode ser    adequado para auferir este tipo de fun&ccedil;&atilde;o executiva (Kenny, Cribb,    &amp; Pellicano, 2018; Kouklari, Tsermentseli, &amp; Auyeung, 2018).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Em rela&ccedil;&atilde;o    ao desempenho do grupo com DL, estes apresentaram escore inferior ao grupo com    TEA na Parte A do MPCR e equivalente ao do grupo com DT, com exce&ccedil;&atilde;o    da classifica&ccedil;&atilde;o em percentil em que os &uacute;ltimos apresentaram    vantagem. Com o controle da idade, o desempenho do grupo com DL na Parte A passou    a ser inferior ao do grupo com DT e n&atilde;o apenas ao do grupo com TEA. O    grupo comparativo ainda mostrou vantagem na pontua&ccedil;&atilde;o total do    teste em rela&ccedil;&atilde;o &agrave;s crian&ccedil;as com DL, enquanto a    diferen&ccedil;a em percentil desapareceu. O resultado corrobora alguns estudos    que apontam problemas no MPCR nas DL e dislexia se considerarmos o percentil    e escore total do teste (ex., Carroll et al., 2015; Salles &amp; Parente, 2006;    Tong &amp; Fu, 2013).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O desempenho rebaixado    de crian&ccedil;as com DL no MPCR sugere a presen&ccedil;a de dificuldades em    intelig&ecirc;ncia fluida, que inclui habilidades visuoespaciais e de racioc&iacute;nio    abstrato. Essas dificuldades podem ser consequ&ecirc;ncia da menor habilidade    e h&aacute;bito de leitura. A profici&ecirc;ncia em leitura tem sido melhor    preditor de desempenho cognitivo que a escolaridade, independente do n&iacute;vel    socioecon&ocirc;mico (Contador et al. 2016).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A dificuldade de    leitura parece ter profundas implica&ccedil;&otilde;es para o desenvolvimento    de uma ampla variedade de capacidades cognitivas, como postula os t&atilde;o    conhecidos "<i>Matthew</i> <i>effects</i>" (Stanovich, 1986) no desempenho acad&ecirc;mico.    Esse efeito consiste no fato de que leitores pouco competentes muito cedo experienciam    maior dificuldade em decodificar o c&oacute;digo ortogr&aacute;fico e come&ccedil;am    a ser expostos a muito menos texto do que seus pares mais habilidosos. Por sua    vez, leitores menos habilidosos frequentemente se deparam com materiais que    s&atilde;o muito dif&iacute;ceis para eles, lidando com uma combina&ccedil;&atilde;o    de defici&ecirc;ncias de decodifica&ccedil;&atilde;o, falta de pr&aacute;tica    e exposi&ccedil;&atilde;o a materiais dif&iacute;ceis. Esses fatores resultam    em experi&ecirc;ncias de leitura precoces n&atilde;o recompensadoras que podem    levar a um menor envolvimento em atividades relacionadas &agrave; leitura, tornando    essas crian&ccedil;as menos propensas a desenvolver habilidades que tamb&eacute;m    s&atilde;o exigidas para desempenhar o MPCR, como racioc&iacute;nio l&oacute;gico    e habilidades perceptivas visuais (Morais, 1996; Nunes, Buarque, &amp; Bryant,    2001). Evitar ler pode ser prejudicial ao desenvolvimento cognitivo global,    pois se pode dizer que &eacute; o pr&oacute;prio exerc&iacute;cio da leitura,    dependente do n&iacute;vel de habilidade atingido, que torna "inteligente" no    sentido dos testes de intelig&ecirc;ncia. &Eacute; por isso que adultos iletrados    t&ecirc;m um QI mais baixo (Morais, 1996).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O desempenho inferior    no MPCR do grupo com DL tamb&eacute;m poderia ser explicado pela hip&oacute;tese    dos m&uacute;ltiplos d&eacute;ficits na dislexia (Pennington, 2006; Peterson    &amp; Pennington, 2015), ou seja, al&eacute;m do preju&iacute;zo fonol&oacute;gico    e na velocidade de processamento, crian&ccedil;as com leitura prejudicada tamb&eacute;m    apresentariam dificuldades em habilidades motoras e visuoespaciais. Em um estudo    longitudinal, Carroll et al. (2015) observam que o desempenho inferior em v&aacute;rias    fun&ccedil;&otilde;es cognitivas, como habilidades visuoespaciais no ingresso    escolar (linha de base), foi preditor de baixo desempenho em leitura posteriormente.    Para Perterson e Pennington (2015), a leitura prejudicada &eacute; resultado    de causas m&uacute;ltiplas, que interagem e s&atilde;o probabil&iacute;sticas.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Poss&iacute;veis    limita&ccedil;&otilde;es deste estudo s&atilde;o o car&aacute;ter transversal,    o tamanho amostral dos grupos cl&iacute;nicos e a impossibilidade de controlar    a vari&aacute;vel sexo. Quanto ao tamanho amostral, &eacute; poss&iacute;vel    que com um n&uacute;mero maior de crian&ccedil;as seja confirmada a tend&ecirc;ncia    &agrave; diferen&ccedil;a observada na Parte B do teste, com vantagem do grupo    comparativo em rela&ccedil;&atilde;o ao grupo TEA. Em rela&ccedil;&atilde;o    ao sexo, o grupo TEA &eacute; praticamente masculino, enquanto o grupo DL possui    predomin&acirc;ncia de meninas. Estudos futuros dever&atilde;o controlar essa    vari&aacute;vel, tendo em vista que teorias do autismo como a do C&eacute;rebro    Masculino Extremo (<i>Extreme Male Brain Theory</i>, Guest, 2016) sugerem que    meninos apresentam desempenho superior em uma s&eacute;rie de medidas de intelig&ecirc;ncia    n&atilde;o verbal e que as crian&ccedil;as autistas seriam uma vers&atilde;o    extrema do c&eacute;rebro masculino. Por outro lado, tamb&eacute;m seria importante    replicar o estudo em meninas com TEA, considerando estudo recente de Harrop    et al. (2018) em que meninas com TEA se mostraram mais motivadas socialmente    em tarefas de aten&ccedil;&atilde;o que os meninos com TEA, todos na faixa et&aacute;ria    de 6 a 10 anos de idade.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Novas pesquisas    tamb&eacute;m dever&atilde;o controlar o n&iacute;vel socioecon&ocirc;mico,    pois uma s&eacute;rie de evid&ecirc;ncias corrobora maior preval&ecirc;ncia    de autismo em fam&iacute;lias com maior renda (ex., Durkin et al., 2017). Por    outro lado, o ponto forte do presente estudo foi comparar crian&ccedil;as com    TEA e com DL em uma medida amplamente utilizada para avalia&ccedil;&atilde;o    de intelig&ecirc;ncia n&atilde;o verbal. Assim, mesmo que o escore m&iacute;nimo    esperado no MPCR (percentil &gt; = 25) tenha sido alcan&ccedil;ado pelos tr&ecirc;s    grupos, &eacute; poss&iacute;vel que as partes A e B do teste possam discriminar    entre autismo e dificuldades de leitura. A replica&ccedil;&atilde;o desses resultados    poder&aacute; consolidar a an&aacute;lise das partes do MPCR isoladamente, al&eacute;m    do escore total, como estrat&eacute;gia cl&iacute;nica &uacute;til na caracteriza&ccedil;&atilde;o    cognitiva desses transtornos do desenvolvimento.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Refer&ecirc;ncias</b></font></p>     ]]></body>
