This article seeks, from the experience report, to discuss the importance of the model of intensification of care for serious cases in mental health. The report gives us the opportunity to look at the complexity of practice in the field of mental health, identifying potentialities and challenges of care in freedom. As a result, it is possible to observe the power of the care intensification work, a practice in line with the principles of the Psychiatric Reform. Faced with the obstacles, we suggest continuing training for the substitute service teams, guided by the guidelines of the SUS and the Brazilian Psychiatric Reform, in addition to pointing out the need to strengthen the mental health policy through critical practice, as opposed to the asylum logic, still present in the territorial health services.
