This article aims to discuss the experience of a clinical case of an adolescent with scarification, based on the understanding of her difficulties with identification models. The methodology followed the presuppositions of a case study in psychoanalysis, with interviews and observation of behavior during consultations, in addition to transference handling. The sessions were transcribed to a list and, later, content selections were made according to the purpose of the study. Based on the reports and the transference relationship, it was possible to identify a symbiotic relationship with the mother figure, permeated by domestic violence and conflicting aspects with a feared father figure. These associated factors were implicated in the impossibility of offering the adolescent a favorable environment for the development of a satisfactory symbolic reordering with her childhood self. As final considerations, this study suggests that it is possible to help the psychotherapeutic follow-up process in the process of adolescence in a context of violence and with gender dysphoria, by promoting a positive bond favorable to the entry of a third element that can provoke sutures in the rupture of narcissistic mandates.
