This paper presents reflections on the diagnosis of psychosis in Psychoanalysis, based on the clinical case of an 18-year-old boy, who was being treated at a teaching clinic in Salvador, Bahia, during the year 2024. His speech was often interrupted, he had flight of ideas and difficulty articulating words, in addition to great alienation from his mother, which proved to be a challenge for the newly graduated psychoanalyst who was treating him to recognize his psychic structure. The diagnosis of Intellectual Development Disorder was subsequently confirmed, which explained the difficulties in speaking and elaborating, but did not necessarily involve psychosis. Listening to his suffering allowed him to be recognized in his desire face his difficulties.
