This article presents part of our research on current symptoms. We have compared the concept of “actual neuroses” in Freud’s texts from the late 19th century in order to highlight the points of convergence with the formation of current symptoms. We emphasize that both the symptoms of actual neuroses and current symptoms exhibit bodily clinical manifestations and result from drive destabilization, a true conflict between the body and representations, indicating a failure in the regulation of jouissance, which, as the Real, erupts and disorganizes the imaginarysymbolic field. Using the metaphor of the grain of sand and the pearl referred to by Freud in 1912 and 1916/1917, where Freud states that “actual neuroses” form the basis of all transfer neuroses, we explore the clinical consequences of this assertion, drawing from Freud’s second drive theory.
