The fear of childbirth can range from mild concern to a phobic fear. In this case, the fear is known as “tokophobia” and can affect the woman’s mental health, as well as her reproductive and sexual choices. The objective of this essay was to discuss the possible origins of the fear of childbirth, its main cognitive and behavioral characteristics, and its consequences for the health of the woman and the baby. To this end, a theoretical study on the fear of childbirth was conducted from an evolutionary perspective, in dialogue with neurosciences and the cognitive-behavioral approach. The fear of childbirth can be learned through classical conditioning, observational conditioning, or information transmission. Cultural factors may further complicate labor and reinforce fear. In addition, cognitions play an important role in maintaining fear and increasing avoidant behaviors, such as the choice of elective cesareans, exposing mother and baby to unnecessary risks.
