The author presents the experience of a psychiatrist in an outpatient clinic for patients with chronic pain. Most of the patients were referred for a psychiatric consultation due to the suspicion of depression that warranted pharmacological treatment, as well as pain already being treated by anesthesiologists specializing in pain management. The psychiatric assessment revealed that only two out of 65 patients had depression. In most cases, it was personality traits or disorders that seemed to explain the pain and/or its perpetuation. Personality traits and disorders have served as confounding variables in the etiological debate between depression and chronic pain. The author presents a conceptual model in which there is no significant precedence between the two conditions; instead, personality traits and disorders predispose to the appearance of both: a depressive phenotype (which does not correspond to the depressive illness) and chronic pain. Pharmacological antidepressant intervention was innocuous in these patients.
