This article aims to analyze, from a historical-critical perspective, the impact of the Brazilian Psychiatric Reform on the training and performance of psychologists in public health networks, highlighting the advances, challenges and contradictions between academic theory and professional practice in the Unified Health System (UHS). This study discussed the main modifications of the exclusionary model of the time, based on the institutionalization of individuals, up to the present period, following the trajectory and adversities of psychologists in the face of the current principles and values adopted through the implementation of the UHS. As it is a relatively recent system, it is assumed that the psychological field continues to grow continuously, aiming at a new perspective of care and attention through health promotion and prevention actions. The results obtained discuss the importance of constantly reassessing the conditions of the care practices offered, breaking elitist paradigms generated by traditional clinic in order to prioritize collective practices.The research is characterized as documentary with a qualitative methodological approach. The search was conducted in the electronic databases of Google Scholar, SciELO, and PePSIC. The interpretation used a qualitative interpretative approach, guided by Minayo, and employed Bardin's content analysis.
