The intensive care unit (ICU) is a place of urgencies and intensity. The lack of flexibility in routines, complications, painful procedures, helplessness, and isolation are common experiences. This study aimed to identify the stressors faced by elderly patients in the ICU and to develop an intervention flowchart. It is a descriptive and inferential cross-sectional study. A total of 185 patients aged over 60 years and with ICU stays longer than 48 hours participated. A sociodemographic questionnaire and the “Stressors in Intensive Care Units – ESQ” instrument were used, and the data were analyzed using JASP software. Based on the findings and the expertise of the author, a psychology-directed flowchart focusing on stressors for elderly patients in the ICU was developed. The flowchart systematized interventions that can be carried out by psychologists and included contributions from other team members. The stressors were categorized as physical, environmental, and psychological, with measures proposed for each domain. The group identified with the highest risk factors included the oldest old, those who underwent mechanical ventilation, those experiencing their first ICU admission, and those with prolonged stays. Despite advances and discussions about the quality of care in the ICU, the need for standardized procedures to improve the practice of hospital psychologists remains evident. These procedures also aim to guide the team’s praxis, seeking to improve patient care and satisfaction. In this sense, the systematization and measurement of stressors are considered significant contributions to the field.
