Patients with chronic kidney disease may experience intense changes in routine, a decrease in physical activities and emotional distress. The diagnosis and treatment may constitute ruptures for individuals, demanding adjustment and adaptation processes. The objective of this study was to explore how the meaning making processes of young adults about the chronic kidney disease are configured and how happened the use of resources in the face of the disease. Cultural Psychology was used as a theoretical contribution from the concepts of meanings, rupture, and resources in the processes of transition. Three young people between 21 and 31 years old, diagnosed with chronic kidney disease and on hemodialysis treatment, participated in the study. Narrative interviews and a sociodemographic questionnaire were used. The participants showed knowledge about with the aforementioned disease and knew aspects about the probable causes of the disease. The diagnosis was marked by fear and uncertainty about the future, but the transition processes allowed the participants to live with chronic kidney disease. Among the resources used in the processes of transition, were the reflections from cultural artifacts; the hospital institutions and their professionals; the family and particular characteristics of individuals.
