Neurocryptococcosis is a fungal infection in the central nervous system caused by Cryptococcus and its main presentation is the form of meningitis or meningoencephalitis, occurring in about 80% of the diagnoses. It shows absence of predictive clinical symptoms and a high incidence of intracranial hypertension, the major cause of mortality. The aim is to report a case of young, immunocompetent, HIV negative patient diagnosed with cryptococcosis with meningeal evolution, bilateral amaurosis, paraparesis, hearing loss, cognitive relegation and intracranial hypertension. The literature on epidemiology, treatment and prognosis was reviewed. The patient was treated with amphotericin B infusion, submitted to ventriculoperitoneal shunting and neuropsychological evaluation, with brain death as outcome. Cryptococcosis is a common infection that may accompany both immunocompromised and immunocompetent individuals without predisposing factors. There is evidence that the approach with neuropsychological evaluation is beneficial and recommended for differential diagnosis, therapeutics, prognosis and rehabilitation.
