Brazil has the largest public organ transplant system in the world, including liver transplants. Patients who undergo this surgery face challenges during treatment, especially in maintaining outpatient care. Treatment adherence is a global health issue, difficult to assess due to its complexity. The WHO identifies poor adherence as a challenge, with no standard method to evaluate it. This study aimed to develop a psychological tool to assess adherence in post-liver transplant patients in outpatient follow-up. The PICO strategy and the PRISMA methodology were used for the literature review, selecting 9 articles. The evaluation protocol developed from this review includes: 1) Understanding of the treatment; 2) Medication adherence; 3) Lifestyle; 4) Psychiatric/psychological history; 5) Social support. The proposed evaluation consists of semi-structured interviews over 2 years, aiming to identify risks for poor adherence and to promote long-term improvements.
