The study aimed to analyze the work organization of healthcare workers with HIV/AIDS patients; identify experiences of pleasure and psychological distress; and investigate individual defense mechanisms and collective defense strategies adopted. The method is exploratory and qualitative, drawing on the theoretical framework of Work Psychodynamics. The data collection instrument was a semi-structured individual interview with eight healthcare professionals; the assessment technique was Nucleus of Meaning Analysis. The study was approved by the Human Research Ethics Committee of both the Institute of Health Sciences (ICS) and the João de Barros Barreto University Hospital (HUJBB) of the Federal University of Pará, under opinions 3,747,021 and 3,912,239, respectively. The results highlight the organization of work with intense and exhausting workdays. Working conditions are deficient in terms of physical structure, equipment use, and resources. Faced with psychological distress, workers utilize the individual defense mechanism of denial and collective defense strategies of rationalization. There is pleasure and recognition for social contributions, gratitude from patients and families, and learning and support from the team. Ultimately, the results provide insightful reflections and contributions regarding mental health in the workplace with HIV/ AIDS, within the field of public health.
