Neonatal Palliative Care is important, as many babies admitted to Intensive Care Units meet the eligibility criteria, but few receive such care. This research, with a qualitative design and interventional nature, aimed to develop a psychological care instrument for neonatal palliative care. Semi-structured interviews were conducted with five psychologists who worked in two public hospitals, and Content Analysis was used to interpret the data. The investigation of the empirical material and the literature allowed the elaboration of the instrument. The results showed the need to include in the instrument: assessment of criteria for palliative care; data on pregnancy and childbirth; information about diagnosis, treatment and prognosis; interaction with the team and bonding with the family; mother's emotional reactions; and psychological interventions. It was concluded that the instrument systematized the psychologist's practice, favoring the welcoming of the family and improvement in care.
