This article is based on reflections and questions arising from the practice as resident psychologist in the pediatric department of an oncologic hospital. There, we encountered children and adolescents impacted by the physical and psychic effects of cancer treatment. Grounded in ethical and psychoanalytical theory, we provide a platform for these beings to voice their experiences, often silenced by medical logic. This prompts questions: what can we offer to these children and adolescents? How can psychoanalytic orientation be validated in the face of the harshness of illness and cancer treatment? So, what can an analyst do when treating children and adolescents with cancer? With the aim of guiding our responses, we present a clinical case as a tool for conducting a theoretical-clinical study, guided by the teachings of Freud and Lacan. The work of mourning and the real of the clinic guide the discussion through an analysis of clinical practice. Returning to theory, we arrive at the desire of the analyst as a function that enables the sustenance of our work in the face of the real of oncology with children and adolescents.
