Stroke (AVC) is one of the main causes of hospitalization and mortality in Brazil an d worldwide. This study aimed to report the case of a male patient, 44 years old with 8 years of education, hospitalized in the stroke acute period due to an ischemic injury to the medial cerebral artery of the left hemisphere, referred for neuropsychological evaluation due to cognitive complaints and mood changes. For the assessment, instruments such as semi-structured interviews, cognitive assessment tests, mood and behavior assessments, as well as hospital functionality/mobility assessments were used. Impairments in declarative memory, executive functions, and anxious symptoms were identified. The relevance of neuropsychological assessment in the acute period is discussed, as it has predictive value for cognitive impairment and possible neuropsychiatric consequences later on. It is believed that hospital neuropsychology may favor early planning of neuropsychological rehabilitation. There is a need for further studies and specific instruments for the evaluation of the stroke acute.
